More and more people are getting worried about whether or not they are predisposed by birth or genetic make-up to Alzheimer's. More recently, I am learning that parents are now worried about their children. \u003cbr /\u003e\u003cbr /\u003eIt is safe to say that the number of people \"worried\" about Alzheimer's disease is growing fast -- currently estimated at 35 million. More than 100 million Americans have been \"touched\" by Alzheimer's disease. This number is also growing.\u003cbr /\u003e\u003cbr /\u003eThis research on the TOMM40 gene is interesting; however, this is early stage research.\u003cbr /\u003e\u003cbr /\u003e\u003ca name\u003d'more'\u003e\u003c/a\u003e\u003cbr /\u003e\u003cbr /\u003eA newly identified gene appears to be highly predictive of not only the risk of developing Alzheimer's disease, but also the approximate age at which the disease will begin to manifest itself, according to researchers at Duke University Medical Center. \u003cbr /\u003e\u003cbr /\u003eThis new gene may be the most highly predictive gene discovered to date in Alzheimer's disease.\u003cbr /\u003e\u003cbr /\u003eIn findings presented today at the International Conference on Alzheimer's Disease, the gene TOMM40 was found to predict the age of Alzheimer's disease development within a five- to seven-year window among people over age 60. \u003cbr /\u003e\u003cbr /\u003e\"If borne out through additional research, a doctor could evaluate a patient based on age, especially among those over age 60, their APOE genotype and their TOMM40 status, to calculate an estimated disease risk and age of onset,\" said Allen Roses, MD, director of the recently established Deane Drug Discovery Institute and the study's lead author. \u003cbr /\u003e\u003cbr /\u003eRoses earlier uncovered the association of apolipoprotein E (APOE) genotypes, particularly APOE4, with the risk and lower age of onset for Alzheimer's disease. This discovery remains one of the most confirmed genetic associations for any complex disease. \u003cbr /\u003e\u003cbr /\u003e\"It now looks fairly clear that there are two major genes -- APOE4 and TOMM40 -- and together they account an estimated 85-90 percent of the genetic effect,\" Roses said.\u003cbr /\u003e\u003cbr /\u003eAPOE4 accounts genetically for 50 percent of late onset cases of Alzheimer's disease but the other half remained a mystery. Genome-wide screening and other new techniques have been used repeatedly without success. Roses' team employed a different approach. \u003cbr /\u003e\u003cbr /\u003eThe APOE gene is present in all people and is characterized by three variants numbered two through four. The Duke researchers found that a variant of TOMM40 apparently evolved independently when attached to the APOE3 version of the gene than it did when attached to the APOE4 version. \u003cbr /\u003e\u003cbr /\u003eRoses said the genetic association with Alzheimer's disease age of onset now goes beyond just APOE4. The researchers found that TOMM40 linked to APOE3 had either short or long repeated sequences, while all APOE4-linked repeat sequences were long. The study concluded that a longer version of TOMM40 attached to both APOE3 and APOE4 are significantly associated with an earlier disease onset, while the short repeat sequences were associated with a later onset of disease. \u003cbr /\u003e\u003cbr /\u003e\"Genome-wide screening detects big blocks of DNA inherited together, but it doesn't tell us all the differences within that block,\" Roses said. \"We conducted a phylogenetic analysis to explore the evolution of the DNA and to see what changes take place on the backbone of other changes.\"\u003cbr /\u003e\u003cbr /\u003eThe technology has not been widely used in human genetics, Roses explained. \"From all the genome-wide scans that were performed over the past four years, it was apparent that the variance within APOE could not account for the extremely high statistical significance which characterized this small block of genes, including APOE and TOMM40, which were inherited in a block.\" \u003cbr /\u003e\u003cbr /\u003e\"If someone gets APOE4 from their mother and APOE3 from their father, they also get TOMM40 as a linked caboose,\" Roses said. \"If the TOMM40 is a short version of the gene attached to APOE3, then that person has a better chance of getting Alzheimer's disease very late, after age 80. But if it's a long TOMM40 they have a better chance of getting the disease before age 80.\" \u003cbr /\u003e\u003cbr /\u003eThe Duke team now plans to validate the association of the APOE genotypes and TOMM40 with age of disease onset and to determine how well these genes predict age of onset. They are planning a prospective, five-year study combined with a drug trial aimed at prevention or delay of disease onset.\u003cbr /\u003e\u003cbr /\u003eThe prevalence of Alzheimer's disease is predicted to quadruple world-wide by 2050 to more than 107 million cases, meaning that 1 in 85 persons will be living with the disease. It has been estimated that delaying disease onset by one or two years will decrease the disease burden in 2050 by 9.5 million or 23 million cases, respectively. \u003cbr /\u003e\u003cbr /\u003eThe research team also plans to use this type of phylogenetic analysis to uncover genetic associations in other diseases, including autism, diabetes and chronic obstructive pulmonary disease (COPD).\u003cbr /\u003e\u003cbr /\u003e--Study co-authors include Michael W. It is distinct from the ordinary mental degradation associated with aging and is a likely precursor to the more serious Alzheimer's disease. Both MCI and Alzheimer's are linked to a steady decline in the volume of the hippocampus, the area of the brain responsible for long term memory and spatial reasoning.\u003cbr /\u003e\u003cbr /\u003eMRIs -- magnetic resonance imaging -- are the most reliable and direct way to detect hippocampal atrophy and diagnose MCI. But for many, the procedure is unavailable or too expensive.\u003cbr /\u003e\u003cbr /\u003e\u003cb\u003e\"MRIs can cost hundreds of dollars an hour,\" Wenger said. \"We created a much cheaper alternative, based on a memory test, that correlates with hippocampal degradation.\" \u003c/b\u003eWenger and his collaborators at the Mayo Clinic College of Medicine, Rochester, Minn., detail their findings in the current issue of the Journal of Mathematical Psychology.\u003cbr /\u003e\u003cbr /\u003eFrom a computer model of an atrophying hippocampus, the researchers determined how to estimate capacity with a statistical measure of how quickly tasks are completed. Applying this analysis to a memory test for people with MCI, the researchers were able to gauge their hippocampal capacity and compare it to the progression of their ailment.\u003cbr /\u003e\u003cbr /\u003e\u003cb\u003e\"My collaborators at the Mayo Clinic backed up this study with MRIs for the MCI group,\" Wenger said. \"These capacity measures we developed showed a reliable relationship to the hippocampal volume measurements, so we know we are on the right track.\"\u003c/b\u003e\u003cbr /\u003e\u003cbr /\u003eThe scientists began by modeling the hippocampus as a complex electrical circuit. Equations governing electric current and voltage mimicked the electrical firing of neurons within the circuit. The researchers switched off neurons in the simulation to model atrophy of the hippocampus.\u003cbr /\u003e\u003cbr /\u003eWith fewer cells available to process electrical signals, the model hippocampus slowed down, but its capacity for processing information decreased at an even faster rate. Capacity was the most sensitive measure of how the hippocampus was deteriorating, more than the average processing speed.\u003cbr /\u003e\u003cbr /\u003e\u003cb\u003e\"We then applied this to the gold standard of the field -- the Free and Cued Selective Reminding Test (FCSRT),\" Wenger said. \"This is a test that can discriminate between normal age-related memory changes and changes caused by impairment.\"\u003c/b\u003e\u003cbr /\u003e\u003cbr /\u003eThe researchers gave this test to five groups of participants: college students, healthy middle-aged adults, healthy elderly individuals, people with diagnosed cases of MCI and a control group of age-matched individuals without MCI. The first three groups each had 100 members and the last two each had 50.\u003cbr /\u003e\u003cbr /\u003eDuring the FCSRT, the researchers showed the participants descriptive words, such as \"part of the body\" and \"artery,\" and asked the participants to choose the picture that fit these cues from a set of 24 images, in this case, a picture of a heart. The psychologists then asked the subjects to recall as many items as they could. For objects they failed to remember, the psychologists provided the category cues, providing more information and testing the limits of the subjects' capacity.\u003cbr /\u003e\u003cbr /\u003eThe researchers analyzed the response times for the tasks and the number of items that were recalled, with and without additional cues. The MCI group showed the greatest sensitivity to added cues -- the additional input either substantially helped or inhibited their performance. But like the computer model, estimates of capacity highlighted the greatest cognitive difference between the MCI group and the others.\u003cbr /\u003e\u003cbr /\u003eThis study's approach to defining processing capacity is unusual. The scientists combined disparate principles of engineering and statistics, mathematically translating processing capacity into what is called the \"hazard function.\"\u003cbr /\u003e\u003cbr /\u003eThe hazard function is well known in engineering, but relatively new for fields like psychology. It gives the probability that a task that is not yet completed will be completed in the next interval of time.\u003cbr /\u003e\u003cbr /\u003eBy measuring how long it takes a participant to recall the objects during the FCSRT, the psychologists fit a model based on the hazard function to each participant and obtain a measure of his or her capacity for the memorization task.\u003cbr /\u003e\u003cbr /\u003eThe difference in hazard function measures between the MCI group and all other groups was statistically much more pronounced than the differences between all groups in the number of items they recalled. These hazard function differences also outweighed the contrasts between all groups in their response times. The hazard function model proved to be the most sensitive diagnostic for cognitive distinctions in the groups, making it a reliable indicator of capacity and a better signal of the underlying hippocampal atrophy than processing speed alone.\u003cbr /\u003e\u003cbr /\u003eThe researchers' results are valid for every person, not just for the whole group.
